Home Language Questionnaire (HLQ)
A completed Home Language Questionnaire (HLQ) form is required for all applications to be considered complete. Because this form is computer-enabled, it cannot be provided online and must be acquired at a DPS school.
Change of Status Form
Change of Status Form (English| Spanish) — Submit the Change of Status Form at your child’s school to do the following:
- Withdraw your child from the district
- Change a school/program within the district
- Change your family’s household size
- Change your family’s household income
- Change your primary address.
