Kindergarten Immunization Requirements

Please provide your child's immunization records to the school secretary at the time of registration. The secretary will make a copy for their records.

Before children can enroll in Kindergarten, they must meet the following Colorado law minimum immunization requirements.

When a parent/guardian obtains a child's immunization document, the document must be kept in a safe place and retained throughout the child's school years.

The child's doctor or personnel in the Public Health Department Immunization Clinic may be contacted for additional information.
